Service Characteristics
We use a developmental approach when writing support profiles. This includes input from clients, families, case managers and employees in relation to health, medication, dietary needs, individual goal setting and other special requirements.

We encourage clients and families to experience Come 'n Try sessions. Emphasis on the client's individual choice is practiced and encouraged by all concerned. We provide an environment that encourages self empowerment.
 


Respite and Home Care, The Elderly and People with Disabilities

Community Choices tailors Home Care solutions for the elderly and people with disabilities, including:

Personal Care assistance
Domestic help
Accompanying clients to appointments
Gardening
Respite care (including planned and emergency respite)
Food shopping
Meal preparation
Provision of food services
Transport
Social support
Companionship
Laundry services
Childcare
Emergency care

Service Characteristics
Community Choices uses a collaborative approach when developing personal support plans, involving clients, their family and other support systems in this process.


 

Day Options

Community Choices Day Options programs run 5 days per week, 48 weeks per year in comfortable, safe and suitable premises with appropriate facilities for supporting people with additional needs.

Programs follow a structured pattern during the day for all participants and offer both centre based and off-site activities.

Our emphasis on promoting independent living skills; encouraging involvement through music, drama, visual arts and crafts, and striving towards social inclusion in local communities for all our participants, forms the cornerstone of our programs.

awareness and use of recreation facilities.
Library access and computer skills.
Art and craft
Music and drama

This program caters for clients with low/moderate support to very high support needs.

Daily Fee: There is a daily activity fee paid by clients.

KLEMZIG

Recreation Services

The following Recreation services are available in your area:

FLEURIEU
A program including recreational activities is available for children and adolescents have a school holiday program which includes activities such as 10 pin bowling, excursions and picnic days, etc.

SOUTHERN SUBURBS
Adolescents have a social program which provides the opportunity to access community based activities on a regular basis. There is also a school holiday program which provides daytime activities for students.

Adults have both daytime and evening programs on the weekend which focus on increasing social networks through regular attendance at local hotels, clubs and events or activities such as excursions and BBQs.

We run a daytime program for adults which provide people with access to meaningful local community facilities and centre based activities.

PARTICIPATION & FURTHER INFORMATION

To participate in the programs a referral is required from Disability SA.
